The facts
Sur reported AITAT dated the start of the sequence at 08:31 on 28 October, when an LZB protection-system fault forced a change to operating conditions; the AVE struck the stopped Iryo about 22 minutes later. [15]
Sur reported the preliminary note described a chain of faults, instructions and actions and said investigators had not yet interviewed drivers or controllers. It is not a final causal or responsibility finding. [15][16]
The reports give different preliminary impact-speed ranges: 160-200 km/h in Sur and 140-200 km/h in Railin. The ministry-hosted AITAT PDF could not be opened in this run, so no single range is asserted. [15][16]
RTVE reported that as of 00:35 local on 2 November, 66 of 69 deceased victims had been identified and 60 families notified. [17]
RTVE reported that a mandatory Renfe circular requires drivers to stop after partial LZB data loss and resume only with controller authorization, then at no more than 40 km/h to a specified signal and after confirming the block is clear. This new operating rule is not evidence of the collision’s cause. [18]
Toledodiario reported earlier on 1 November that a protocol was being drafted; RTVE’s later report describes the mandatory circular as issued. Neither report establishes whether or how this procedure relates to the crash. [18][19]
EFE and Europa Press reported that the Council of Ministers approved €30 million in assistance: €15 million in direct aid and €15 million in advances through insurers. The reported award is €72,121 for each death and €2,400–€84,141 for injuries. [2][3]
EFE reported that the transport minister announced an immediate national rail-safety plan, including additional system checks and operational changes. These are announced measures, not a finding about the crash cause or confirmation that every change has been implemented. [3]
Relevant context
Renfe identifies its affected train as the 08:00 Madrid Puerta de Atocha–Sevilla Santa Justa service, scheduled to arrive at 10:39. These are the journey's scheduled local times, not the crash time. [6]
Still unconfirmed
- The final casualty totals remain unsettled; the dated regional inpatient count is not a reconciled total number injured.
- The final technical cause, exact impact speed and responsibility remain unestablished. The minister's reconstruction and technical reports are preliminary.
- The welfare of the two Australians receiving consular assistance remains unconfirmed.
- AITAT's final causal findings remain pending. Sur and Railin report different provisional speed ranges, and the ministry-hosted note could not be directly retrieved for comparison.
- The final technical cause and responsibility remain unestablished; the new Renfe operating rule does not establish a causal link to the collision.
- The cause of the collision remains unestablished in the sources reviewed for this update.
Corrections & updates
Update: Added the preliminary injury figures and the government's breakdown of deaths by train, reported by CMM and Europa Press. The official provisional death toll remains 30; final casualties and the cause remain unconfirmed.
Update: Raised the provisional death toll from 30 to 64 following new government figures reported by Europa Press and CMM. Replaced the earlier preliminary injury figure of at least 80 with the conflicting totals now reported: 58 by CMM and 80 by Europa Press. Final casualties and the cause remain unconfirmed.
Update: Raised the provisional death toll from 64 to 68 and added García-Page's latest injury triage figures, replacing the earlier conflicting overall totals without asserting a final injury count. Added Puente's preliminary account of the safety-system failure and restart decision, plus Renfe's original service confirmation. Final cause and responsibility remain under investigation.
Update: Added a detailed timeline attributed to Transport Minister Óscar Puente, including the LZB failure and the control-centre authorisation, and linked Spain's official mourning decree for 29–31 October. The investigation has not established the final cause or responsibility.
Update: Raised the provisional death toll from 68 to 69 using the regional emergency service and Servimedia’s later check. Added the dated regional hospital count and aligned the approximate collision time with the investigation record cited by Servimedia. A briefly published investigation entry of 74 was corrected to 69; it is not the official toll.
Update: Added 30 October DFAT consular-assistance information reported by ABC and 29 October recorder-recovery information reported by Cadena SER. The official provisional death toll remains 69; the two Australians are not added to any casualty total. Exact speed, final cause and responsibility remain unresolved.
Substantive update, 1 November: added reporting on AITAT's first 24-hour note, including the 08:31 LZB fault and preliminary chain of events. The note is not a final finding; Sur and Railin publish different preliminary speed ranges, so neither is presented as definitive.
Substantive update, 2 November: the death toll remains 69; RTVE reports 66 victims identified and 60 families notified. Added RTVE’s later account of Renfe’s mandatory LZB-loss procedure and a local report of the earlier draft stage. The procedure is reported as a safety response, not as a causal finding.
Substantive update, 3 November: added Cabinet approval of €30 million in assistance for victims and the transport minister’s announced rail-safety plan. The death toll remains attributed at 69; neither the aid nor the safety announcement establishes crash causation or implementation.
